Price per ticket (indicate all prices for example, 1 for $2, 3 for $5, 8 for $10). Maximum individual ticket price is $2 for class D licenses

Please describe the prizes and value of each to be won. Max value per prize is $500. If more than 1 list them all. If multiple of the same prize just list it as one with total value (i.e. gift cards $500)

What is the total cost of the prizes if not donated? Purchased prizes cannot exceed 50% of the projected gross revenue.

Use of the proceeds (explain what the proceeds will be used for). Proceeds should go toward player development (i.e. ice time, tournaments). Items like jerseys, swag and will not be approved.
